Eligibility - Homelessness Prevention

  • Individuals or families facing eviction.

  • Resident of Golden Valley, New Hope, Crystal, Robbinsdale, or Plymouth (east of 494), or referred by one of our partner agencies within the northwest suburbs of Hennepin County.

  • Have used from Hennepin County Emergency Assistance (EA) within the past year, or have currently applied for EA and been denied.

  • Income guidelines apply. Contact PRISM for questions regarding eligibility.

Homelessness Prevention Program

Each year we help 250 households avoid eviction by providing housing-related financial assistance, advocacy, assistance with applications for energy assistance programs, and referrals for community resources to address other issues such as transportation, employment, child care, and mental health care.


Prevention participants may also access our Marketplace Food Shelf and Shop for Change Thrift Shop as they work to regain stability.
